How to Apply for Insurance Coverage: 

1.  Ask your doctor for a prescription written as “cranial prosthesis.”

2.  Keep all documentation (diagnosis, prescription, treatment plan). 

3.  Request an insurance-ready invoice from Grace Haven. 

4.  Submit your claim to your insurance provider for reimbursement.
